EPF WITHDRAWAL

Sir,

My EPF A/C No. OR/6296/5. I have applied for full settlement of my contribution on dated 06-10 but till date not settled by the Regional Office EPF Bhubaneswar.

Bhavishyanidhi Adalat passed the Order vide No. 100404 dated 12-04 for payment of Rs. 9441/- & Rs. 2870/- but department not paid till date.

My total contribution is Rs. 38668/- from 1998 to 2006 including employers share but department only credited my EPF account for the year 1998 to 2000. In-spite of my several followup no action taken by the department only inform that employer not submitted Form 3AR & directed to the employer for submit the same since 28-09 as per The Samaj Adalat.

You are requested to kindly look in to the matter & take appropriate action to settle my pending claim from 06-10.
