Mis-place of the request form

I have submitted PAN card form (49) at the E-Sava center rati bholi hyderabad 500037 two times and both the time they mis-place my form and they did forward it to the PAN card office for the processing and when I was get back to the center to know the status on the request, the lady manager starting the filthy language and did not provide me any information on my request and when I have asked to show me the records when they forward my PAN card form. They don't even have any records on my request.

Then, I have contacted the PAN card customer service, this people informed me that the E-Sava center did not dispatch my from to the PAN card office and still they do not receive the PAN card form.
