ESIC gurgaon/ESIC Card
Not send my ESIC Card

Respected Sir,

My name is viks Saxena I have make a esic card my Insurance No-6921367535 and Employer code no-69000305110000305 I am going to esic office for photo attach through camra for the month of Nov.2011 but I have not received ESIC Card till date my Esic Card .i am going to ESIC Office many times (office Herohoda chowk and Raajiv chowk) but he can not give my card and not give me satisfaction answer.so please arrenge my card as soon as possible
