Incorrect entries in the identity card

Please take urgent action to get my identity card bearing No. HNJ 2401248 incorporating the following corrections

1) voter's name > C. Monickathas (as per secondary school leaving certificate)

2) Age as on 1.1.2006 > 55 (54 completed)

3) Address > Door No. 7 (new). 89 (old)

10th Cross Street,

Neighbourhood Colony,

Tirunelveli 627007

4) Municipal corporation ward No > 26

Sir, I have furnished above the correct particulars.

I am constrained to furnish incorrect name in each every document in my day to day activities because copy of the voter identity card is required by the authorities concerned. Hence I request Election commission of India to be kind enough to issue a revised identity card, please.
