Not getting p.f slip for last 2 yea

Dear sir,

we are the employs of New victoreya mill & from 9 to 2010 we have not got the our P.f slip so we would like to know why have you not sent the slip.your organization is doing work in a proper way.please take an action about this matter & would like to inform you also that mill had also taken an action about this matter but from side no response came out.

Kindly take a action about this matter.
