PF Form 10C Withdrawl

My PF account details - State: MAHARASHTRA / Region: BANDRA / Establishment Code : 18734 / Employee No : 17571

Dec 20 - PF withdrawl form-19 and from 10C submitted to pf office by Patni.

Mar 04 - Due to some issue it was returned back to Patni. Patni then again submitted my form. Then, I received claims for form 19, but I haven’t received claims for form 10C.

Mar 04 - Requested PF office to check status for from 10C.

Mar 11 - Application has been returned (2nd time) to Patni on 12.01.10, kindly contact them for resubmission.

Mar 12 - Requeted Patni to check

Aug 16 - Patni has not received form back and suggested to fill new withdrawal form.

Oct 13 - New form 10C again sent to Patni

During above period I had enormous number of phone calls, email communication and a registered post communication with PF office and Patni.
