SUB: Non Disbursement of Pension sanctioned to me under EPS-95, PPO No. 1021-reg.

This is to inform you that my pension payment has been transferred to the HDFC Bank, Sarjapura Road Branch, Bangalore, in 2006 and I had been getting the same till Dec. However, this has been discontinued from Jan inspite of having submitted my life certificate to HDFC Bank, Sarjapura Road Branch, Bangalore in Nov.

A copy of my account statement is attached fyr. You would note in the statement that the last pension of Rs. 841/- was credited on 02/01/10 for Dec and there is also a credit of Rs. 841/- on 07/04/10 for Aug..

Request you to look into the above and continue my pension disbursement with arrears wef Jan.
