DELAY IN EXECUTION OF FORM 13

Dear Sir,

My Husband Late shri Ranveer Singh Shekhawat has died on 12 May-11. I have submitted FORM NO. 13 (REVISED) on 05 Sep-11 for transfer of PF amount from my husband previous company Mother Dairy Fruit & Vegetable Pvt. Ltd. Goregaon East Account at PF office Kandivili to Frigorifico Allana Limited Fort Mumabi Account at PF office Bandra. I am regularly following up with the kandivili office Group-65 office clerk Mr. Manikant. But my paper has not moved from his desk for 4 month then I have written letter No. MUM/MDFVPL/HR/1612 dated 11/02 to Asst. Commissioner Kandivili office. No result from the letter again visited on 6th March-12 and requested for early action Mr. Manikant has taken out letter and inform that in one week it will be send to Bandra PF office after following up 2 time again when I contacted him today he has same reply that paper is not signed and it will take another week.

Six month has passed after submitting the form initial process has not started. My husband is no more and I am depending on his pf amount and pension to run my family. I am facing the hardship even after mentioning my problem your office staff are taking it in very casual.

Kindly under stand my status and execute my PF form as earliest.
