Non-receipt of claim amount in my PF account

Dear Sir,

This is to bring to your kind notice that I have submitted my PF claim Form 19 for withdrawal of money in my PF account through my ex-employer M/s. Egis India Consulting Engineers Private Limited, Faridabad on 28.11.2008.

After some days when I enquired about my claim status, I was informed that the application was returned for unknown reasons and sent back to my address given in the application. But I did not receive the same till date.

In the mean time I have resubmitted another form 19 again through my ex-employer M/s. Egis India Consulting Engineers Private Limited, Faridabad on 27.05.2009 vide your office acknowledgement no:22040773.

my previous employer’s representative visited your office in the month of September and enquired about the status of my claim and it was mentioned that the claim will be disbursed with in 10 days time. Till date I have not received my claim amount.

Hence I request you to look into the matter and clear my claim at the earliest
