Locking of sim due to verification at wrong address

Sir I have taken one add on connection (No.8427007620) to my existing cell No.9876736858 from ARC Sector 37, Chandigarh and submitted all the required documents. But after 02-03 days the outgoing facility was blocked I contacted at 198 and they told that verification is negative and they again got verified but the outcome was same. Then I again inquired at 198 and I was surprised to know that the verification was done at some address in sector 18, Chandigarh whereas I have never ever lived there i.e. my address was changed without my request.

You are hereby humbly requested to get my antecedents verified from the correct address i.e.#788, Sector 41-A chandigarh and arrange to get my second No."8427007620" started immediately to stop further harassment to me.
