To deactivate the sim

Dear sir,

I lost the BSNL SIM Card with my cell phone recently at the West of Chord Road, Rajaji Nagar, Bangalore, dt.6th March at 9.15am please block my cell no: 9481473734 issued at BSNL, Vijaya Nagar, Bangalore. Which is issued on my Name i.e. Prashantha. S.

Please issue a replacement SIM card. Let me know where to collect it and what are the charges?
