Deactivation of Mobile Radio Service

My BSNL Prepaid mobile number is 9490964899 I am being charged Rs.25 on 30 th of every month for mobile radio service, never subscribed by me. Every month I am getting message from 1263001 and also Rs.1/day. I have taken the help of customer service on mobile several times and I am given different procedure everytime for deactivation but none works. Please help me. Ramanaraju
