Unauthorized activation deduction of balance

Dear sir

my mobile no is 9415678361 .the service provider bsnl activated masti pack from 565054005 on dated 09.10.2010; 20/11&20/12 without my consent and deducted Rs 05 per mandatory subscription. It is cheating &crime against my service &privacy. So I want deactivation& refund my balance.
