NON-RECEIPT OF PUK CODE ALONGWITH THE SIM

I've purchaged a SIM (clled BSNL Family Pack SIM No.8987415206) on 27th Nov from BSNL, CTO Buildg, Sector - 2, Bokaro Steel City - 827001. But, very important code said PUK Code have not provided by the service provider causes a lot of problems is being facing due to that.
