Disconnection of Prepaid Numbe

I have taken one Airtel Prepaid Number in the month of Jan but after 3 months (April) its got disconnected on (04th April), I have taken this Prepaid Number after giving all necessary documents, but now company is saying that I have not submitted the Form. How is it possible, that without proper Form I was using Airtel Number for Three Months. I was also a balance of Rs.450/- in this card but now the company is saying that I will not get this Balance and number is disconnected.
