Dear Airtel Service Provider,

I have lost my SIM Card bearing no. 098953-98644, a few days before while i was in Bangalore. Hence i kindly request you to block this sim card.

Kindly notify me in case any other formalities have to be catered to and it would be of great help if im made aware of further details to obtain a duplicate sim card.
