Avia life Insurance

Hi,

I joined in avia life insurance. Now i thought to surrender my policy. Now i asked to avia life insurance and their respective bank (Canara Bank) for policy surrender form. But from the past 2 week they didn't respond and they are escalating it. Where should i complaint about this?

Please help me.
