PF Settlement

My name is Parvind Sahay was employed with Aviva life insurance for 21st march to 3rd january. I have sent the pf form duly signed and stamped on 6th sep 2010 for settlement and was told to check my bank account within six months to have the settlement done. till date nothing being done and on rigourous follow up no with the company ther is still no confirmation on the same. on contacting the epfo north for the same i came to know that no form being submitted against PF No - DL/25803/2 which is my pf no.

Pl help and suggest me that what should i do in this case. i have given a complain mail to epfo online email address but no confirmation on the same.
