Unathorised Transaction

I am having salary account with axis bank, there is a transaction showing in my statement of Rs 500 on 05-06 withdraw form KANDIVALI HDFC ATM but this transaction is not done from my side, i had given one application letter to KALINA BRANCH on 10 june and also i spoked to cutomer care execative but they are not helping me out, Kindly do the needfull or suggest me the right way
