Car insurance policy car no-DL4CAJ8872
ORIGENAL CAR POLICY NOT RECIVED

I had applied for a car insurance from agent Kavita Sharma, agent license code 110927, agent contact number 011-25289933. My car insurance expired on 04-10. I had given her the details of car along with my current residential address on 22-09. I have paid an amount of Rs 12000 by cheque no. 648508 drawn on Axis Bank Ltd., vide reciept number W 001812429 on the same date.

Till date I have not received the original insurance papers and the address mentioned on the xerox / mail copies sent to me are wrong. On bringing it to the notice of the agent, I was told that I will have to re apply the whole form and that nothing could be done. Inspite of the mistake being on the part of my agent I am suffering.

Please look into the matter and do the needful as soon as possible.
