Non receipt of payment

Dear Sir/Madam,

I am brother of Late Vinay Kumar Rai who's life insurance policy was taken from HDFC standard life insurance Policy Number 12119077.(date of registration was 29 Aug).

2. My brother Vinay Kumar Rai was decleared dead on 01 Jan at AIIMS Ansari Nagar Delhi.

3. After declearation of dead i have informed to Union Bank of India because education loan was sanctioned from Union Bank of India, and it was forwarded by UBI to HDFC for the same. Death certificate was issued by New Delhi Municipal Council death certificate Number 71787 and it was forwarded to HDFC life Insurance company on 13/05/09 at HDFCS. LIC, Seth Shopping Complex, Ghazipur (UP) and second copy was submitted to the same place on 13/08/09. A copy of duly filled in Doctor, s Hospital Certifiate vide letter No. F4-IV/Hosp (MR)/183 from AIIMS Ansari Nagar Delhi was forwarded to HDFC Staandard Life Insurance company New Delhi.

4. It is requested that Action may be taken in view harshnent of my Family.
