Forged Documents

I took a policy from HDFC Standard Life Insurance in the end of February. It took 3 months to get the policy documents. When I got the policy documents, I found that, at many places my signatures are forged in the policy application. The contact details given were totally wrong. I made a formal complaint on 29th June, and as of today I have not received a resolution. Because of the errors, I am not able to login to their system and verify my policy details. Talking to their customer service department for last one month and the matter is not resolved.
