Nin availabilty of surrender form on co. website

I want to surrender my Life stage Pension plan policy. I live in Dhar (M.P.). I purchased the policy through sales executive available at ICICI bank brach DHAR. Now I want to surrender above policy. I have approached the branch for getting policy Surrender form. But since last more than 15 days I am regularly being turned back without form for one or the other reasons.

I searched the form on company's website also but it was more shocking that it was not available even at the official website. This indicates wrong and totally unfair practice of the company which not at all expected from the company of international repute. I have fedup with the co. at very first stage of surrendering the policy..
