Not getting my Policy Bond

Dear Sir,

I have registered my Insurance policy in Challakere branch (BO-62L BRANCH) through an agent by name K.N Shivananda (agent code 15403790) in the year 2005 still from that date my premiums amount has been deducted from my salaree but still i have not collected my Policy bond the reason is my agent upscand by some where i dont have any contact with him. in this regard i have written so many requisition to concern branch but they are not replaying for my request please help in this matter.
