Health card not received

Sir, i have purchased in year Health Plus policy No-255135302 policy holder-SUNIL DUTT YADAV from LIC OFFICE DAHMPUR, BIJNOR, U.P., but still iam waiting for health card (TILL DATE NOT RECEIVED HEALTH CARD).

i have deposited all three (3) annual premium to LIC, And I have submitted request & written compaint, two times, many times telephonic talk, personally contact to the LIC DHAMPUR BRANCH, OFFICE.but all in vain.

So, pl. may look in to the matter & instruct to the concern, & may oblige me, i shall be highly thankfull to you for nice coopration & n/a.
