NON-RECEIPT OF ORIGINAL POLICY BOND

Sir,

1. I had taken Life Insurance from LIC of India, Mhow Branch (342) with Policy No. 344204671 with Date of Commencement 12-10.

2. It is regretted to inform you that despite repeated postal letters issued to LIC of India Branch Office Mhow and verbal requests made to Sr Branch Manger, LIC of India, Mhow Branch, I have not received my original Policy Bond till date.

3. It is requested to kindly instruct the LIC Branch Office Mhow to send me the Policy Bond at the earliest please.
