Premium Payment online - Forgot User ID and PW

Sir/Madam,

I am V. SAVITHA holding LIC policy No.713398575. Premium for this policy is payable every 6 months by 28th March and 28th September. Due on 28th September was paid by me using online facility. Unfortunately, I forgot the User ID and corresponding Pass Word.
