Dear Sir,

I have got a policy paid receipt from your agent Mr. Md. Eajaz Ansari (Agent Code No. 07104431) for my new policy No. 428323600 vide date of commencement 26.02.2010. Due to some unavoidable reason I do not like to this policy. So within your trial period I reject this policy.

I do not receive yet my policy bond. Either you do not issue the same then cancelled the same & return my full money or issued your policy bond certificate then refund my policy money after deducted stamp charged.
