Sir,
I was taken a policy departmentally in MAX NEW YORK LIFE under plan LIFE MAKER GOLD with policy number 446143489 but consequent on completion of locking period I surrendered the policy in Dec but despite repeated and several request the company not turned up on his promise your partner for life and we care - in word as well as deed. I have requested several time through registered letter and making complaint over their service help desk number and e/mail but still no cheque received as requested. I"m requesting since surrender the policy to send the cheque on my following official address :
