Dear sir/ Madam,

I have successfully completed training program for LIC agent and also qualified in the online exam in the month of sep 2011. However, my license has not been released till date because in the month august, I myself applied with Max new york life insurance branch- astral heights, 7th floor, road no - 1, banjara hills, hyderabad vide urn no. MAXI 2207110015 but I have neither attended training program nor appeared in written exam for Max new York life insurance. In this context, I approached Max new York life insurance branch, road no 1 jubilee hills for cancellation of urn no issued to me by them & I have also mailed them regarding this issue, but I didn’t get any response from them.

So in this regard I need your help please help me for cancellation my urn no. MAXI 2207110015 as soon as possible so that my license for LIC agent can be released.
