Non receipt of pli bond certificate

Sir, I have opened a PLI through your good organization bearing proposal No – 808764

/797 /EA /45 & the amount of Rs. 435/- per month is deducted from salary through CP bill from the month of sep 2006 and being continued.

2. Even after 5 yrs, subscription of premium the concerned policy certificate alongwith connected documents is not yet received by me.

3. Ref your letter No. Misc/BSF/APS-7D/III dtd 07 Aug’ 08, I have written to times to PAD BSF N/Delhi showing an outstanding balance of Rs. 4350/- due for payment to ADG, APS (PLI). But it has been more than one yrs since the last letter, I was sent to PAD, I have received neither any intimation regarding payment nor Bond certificate from ADG, APS (PLI) cell.

4. It is therefore, requested that the original Bond certificate of PLI No. 808764 in r/o Dipak Kr. Saikia may please be sent to my under mentioned address at the earliest for which I shall remain grateful to you and your department otherwise please inform me further action by return of post.
