Loss of ATM-cum-Debit Card

Dear Sir/Madam,

I am to inform that I am pursuing my education in Pune, Maharastra since 2008. Suddenly, I lost me Corporation Bank and SBI, ATM-cum-Debit card on 05.01.2013 somewhere in Corporation Bank, Hadapsar Branch, Pune while carrying out some transaction in the Corporation Bank.

I request you to block my ATM-Cum-Debit Card transactions with immediate effect. The Account details of both the banks as follows :
