Harassment in Paying PF withdrawal amount

Dear Sir/ Madam,

I (Jayesh Koriya) was working in AVIVA LIFE INSURANCE CO. LTD. from

Aug to Feb as FPC (Financial Planning Consultant), My Employee Id being, 9189. I've resigned

from the duty on 29th Feb. I am following up for my PF withdrawal amount from last three-four months. I have also submitted the required PF Withdrawal form to your office through local SM Mr. Karan Sukhadia @ surat aviva office, who had helped me getting it filled up. We both asked local staff for the completion of the form. But, somehow it has been rejected from your main office. So, i'm asking for your help now. What I suggest is that you please intervene in this matter So, that I don't have to wait & be furthermore harassed for my PF amount...

So, I request you to help me in this manner as soon as possible.
