PF refund

I have worked with Inteletenet for almost 4 1/2 years. My employee ID was 958 however till date I have not recevied any intimation on my PF status. Is there some authority in the HR who can work out a solution for the same.

A letter from my ex- employer but there was no response from there end. Please find the attachment for your reference.
