LOST MY CARD EZ-PAY CARD

SIR,

My name is KARYAMSETTY SUNDEEP.I am studying 3rd year (EIE) in BAPATLA ENGINEERING COLLEGE.I have my lost my ez-pay card since a month. The card number is 4216870112945090.The PIN NUMBER is 9800. and the web pin number iS 8Q5FR5XY.Though i had the card with me it was not activated properly. Almost all our students had got the money but i never received any money in my account.

So please kindly look into this matter and hope you would issue me a new card.
