Not signing the P F settlement form

Dear Sir,

I was employed with M/S Bhuwalka Trading & Tea Co. Pvt Ltd, 135 B R B Basu Road, Kolkata 700001. I have resigned the from the company since December.

But the employer is not signing the P F settlement form in form no 10C & 19 even after repeated request. I would request you to look into the matter & take action against the company & ask them to sign my P F claim form. I will be waiting for your reply.
