Sub:-Compalin against Harrashment and Negligence By ESIC (Budge Budge) Kolkata westbengal

Respected Sir,

My Name is Surajit Samanta, I lived in P/V-daulat pur, P/S-Maheshtala, Kol-139 My Near By ESIC SD-NUNGI-5093 My Insurence No.41-12746346, I am contributing My ESIC For 2.6 years but still now I wont' got Any Treatment Or Any Helped From them.

1.My mother Was still Ill, She is Suffering From High Sugar but She won't Get any treatment

2.One year ago A accident Occur and my Leg was Broken They refuse me and sent Me to Maniktala kolkata head office, I got Treatment from them. Even Doctor Advise me to Take rest For Three month and they advised me not mooving not walking, they suggest me to take totaly bed rest after I Recover I won't get any Piece from them.

3.When I was tranfer to kolkata maniktala they just give me a titenus not a pain killer

4.I Have Smart ESIC Card Now, 1st time I Want to Treat my mom at SD-NUNGI-5093 They asking me for form 7B

and Form37,

5.Now I when I submitting My ESIC Smart Card form7B Form37 Still They Refuse me and order to take a TEMPRORY CARD

Now I Have Question Can I Got my Treatment From Here?
