I have take Health Advantage Plus from ICICI LOMBARD in the year 12th Jan and I have purchased it from ICICI LOMBARD online and I also mentioned by pre-existing diseases from the option available and I have also mentioned year since when I have had the disease and I also mentioned my exact diseases in other option but that is not mentioned which they gave policy. After paying third year premium (Rs15000 x3) I suffered from a disease so I required the claim but then they refused to give the claim because I have not mentioned the exact disease but there was no such option having my exact disease on the website so I choose the closest one. My policy no .4063/hab/w-081482/00/000 from 12th Jan to 11 Jan second year policy no 4063i/HAP/W-486094/00/00 DT. 11th Jan to 10th Jan Third year Policy no is 4063i/HAP/W-486094/01/000 11th Jan to 10th Jan.
