Cancellation of ATM Card

Dear Sir,

I was in receipt of ATM card in the month of Jan., 09.I contected my bank but they were in not receipt of PIN NO. During this I have lost/misplace the envelope of ATM card. The ATM card was issued agt. my SB Account No. 10054370713 with SBI Malviya Nagar, jaipur, Now you are requested to kindly cancell the ATM card issued earlier and issue a new ATM Card at your earliest and inform me accordingly.
