Gear slippage and clutch hard

Sir

this is a complaint which I am facing from the day one that my bike's gears slips at 4 th gear oftena and the clutch becomes hard after 500kms from service. I had complained the same to Authorised dealer about 05 times but they just do eye washing job now my bike has covered around 10000kms

pls i want an action on this complaint immediately
