Dear Sir/Madam

My brother's residential phone number 0522-2432657 has been lying dead for the last 10 days or so. My brother is a cancer patient and in the absence of the phone, he is not able to contact his doctor or SGPGI, Lucknow. We have lodged at 198 service and to all possible officers but no action has been taken as yet.

I request that the said phone is restored at the earliest.
