Cancellation of health insurance policy

Sir,

Subject :- Request for cancellation of Reliance Health Insurance Policy No 28251047600- R.Mahendran:-

I wish to lodge a strong protest against the your esteemed company which has cheated me by issuing two policies to my family without obtaining my consent.

1. Policy No 282510475716 was issued in favor of my wife Sandhya Mahendran by deleting my name as head of the family.

2. When a request was made for correcting the policy, the call was received from phone no 11140784947 on 6.7.2009 at 20.33 hrs assuring of the correction and inclusion of my name in the insured list.

3. On the other hand after IVR PHONE conversation, Rs 12216/- was debited from my ICICIC CREDIT CARD NO 5176 5335 1994 1006 without my consent inspite of reminding the call centre executive that no further debits are liable to be paid by me. This has resulted in issue of Two Policies for my family by deceit and cheating. The conversation recorded at 20.33 hrs to my mob no 98450 63784 may be verified to confirm the above facts.

In view of this bitter experience, I am constrained to cancel the policy no 2825104 76000 with immediate effect.
