Dear Sir/Madam

I have personally submitted my cheque request leaf duly filled & signed by me at yr sakinaka branch. ratherthan delivering the cheque bk in person, I hv been told that it will be couriered to my address. I'm sorry to say that as on date I hv not recd my cheque bk, it is almost a month now.

request u pl lookinto the matter & arrange to courier my chqbk at the earliest & oblige.
