Extra billing and unnessery harrasment

I had purchased the mediclaim policy for my family via the credit card of citibank and policy was of 7128/-ut they charged me of total amount 10584- giving me some accidental policy plus service tax on each members name which was a group policy

later on i cancelled the policy after this thing but these people are sending me regular notice for payment and i have so many times sent the request to them fia fax and mail and the policy has already been cancelled and still they are calling me and sending bills for the same and i have never applied or made a request for any kind of cards to these people they had send them without any request and now not cancelling them and sending regular monthly bills kindly take some necessary action regarding this issue
