I Mrs. S.Kusuma kumari Worked as a teacher in D.A.V PUBLIC SCHOOL RAJAHMUNDRY from 06.6.2006 to 14.6.2008.I have resigned on 15.3.2008. I have submitted in my C.P.F.Settelement form in D.A.V. Kovvur School on 15.7.2008 (dur to Rajahmundry School closed in june'08) But till date I have
not recd my P.F.& E.P.S. amounts. Kindly arrange to settle the same immediately.
