Without information Cut the name from Voter ID Card List

Respected Sir,

My Name is - Madan Umakantrao Dusane, I'm Living & working in Bhopal from last 20 years & I have a voter Id card holder from 1995 & my card no. is - "MP/30/239/381042" & my Res add. is EWS-563, Kotara Sultanabad, Bhopal...

But, Here is the problem is this year i can not vote b'coz My Name is Delete from Voter List. How make this mistake I dont know. Now I'm living in E-8/36, F-102, Shyam Homes, Trilochan Nagar, Trilanga, Bhopal This is my Own House & last time I Fill the Form of Voter ID Card for my Wife (Name is- Mrs. Vandana Madan Dusane) but her name also Not in list of Trilanga Area

Now I want Both of ours Name Add in voter ID list and Election Office, Bhopal were Issue the Voter ID Card to Us & We are also Chose our 'NETA' for us

so, Guide me what should I Do...
