Statement not received

I have 1st applied for the bank statement which i required from 01.04.2008 to till date of application around 26th Nov.2008 but after 15days i didnt aget the statement so that i have again went personly to the paldi (ahmedabad branch) & aplied with wriiten form on dec.2nd week but than Rs.112 was charged from bank in my acct for this but i didnt receive the statement so again i have complained from the bank online from the same branch but still i didnt reciev the statement of my account so again i complained through phone & my compl No.was #123365 but still they have not sent me the account statement. from 01.04.2008 to 31.12.2008 which i had requested in jan.2009 & 10.02.2009 also also.
