Non receipt of policy

Dear Sir,

With reference to above, this is to inform that undersigned has submitted the form alongwith necessary documents and cheque no. 661517 of Rs. 1500/- dated 27.12.08. However, till date I have not recieved any policy paper or any other communication except reciept of Rs. 1500/- paid by me.

Reciept issue date is 29.12.08, but till date no policy has been recieved. when I inquire in mid of Feb with Mr. Vivek on Mob No. 9825494147 your executive, i was informed that due to pending of certain papers policy was not issued and it has been kept on hold. I haven't recd. communication from your side regarding providing the documents.

Just now I had call customer care and spoke to your executive Ms. Anuradha Reddy and has been informed that policy has been issued on 23.02.09 and the delay was due to fulfillment of certain parameters.

Kindly give me the reason for such long delay further explain which are the parameters that requires two months time to comply.

Further, I hereby request to cancell the above policy with immediate effect and refund the amount of Rs. 1500/- which i had paid initially. Further, please note I had opted ECS payment for insurance policy. As I not satisfied with the service and not interested to continue with the policy no ECS should be debited from my A/c.
