Cancellation of my health card

I dont need any type of health card taken in my mothers name with her DOB 25, feb, 1964 and the amount has been debited in my credit card card account. I just want cancelltion of that card and credit of that amount. Just this is harshing on me. Iam in such a position that I cant pay any amount towards this card.
