Non receipt of documents

Dear Sir,

I regret to inform you that I am yet to receive the the documents pertaining to the Health Insurance Scheme although the charge has been bedited to my account.

Inspite of my repeted request to esnd the documents in time for Incme Tax purposes I am yet to receive the same.
